Please list your PRIMARY insurance. If you have BCBS, Humana, Aetna, Amerigroup, Ambetter, CareSource, PeachState, Medicare or Tricare. If you have another insurance, we can offer you a superbill to submit to your insurance company. Our practice does not submit to secondary insurance policies unless both primary and secondary are one of policies we accept.
